Sullivan Nicolaides Pathology:
- Lost or destroyed two of my blood samples.
- Provided very significantly erroneous results for vitamin B12 and homocysteine.
- Cite a very significantly low "normal" range for vitamin B12.
- Sent out three samples frozen in the wrong type of tube, invalidating the results.
- Sent out my samples to Mater Pathology for MMA testing for B12 deficiency.
- Failed to investigate when I complained about Mater Pathology.
Mater Pathology:
- Provided very significantly erroneous results for Methylmalonic Acid.
- Took up to 35 days to produce results when tests are performed weekly
- Provided erroneous documentation with samples sent out to other labs
- Ignored my request for repeat of tests, at my expense, and destroyed samples
- Destroyed another four samples that I had arranged to be re-tested by another lab.
- Threatened me with legal action
Queensland Health Pathology Service:
- Provided significantly erroneous results for methylmalonic acid.
- Took up to 36 days to produce results when tests are performed weekly.
- Their reports did not contain any comment about the excessive methylmalonic acid levels.
Pacific Laboratory Medicine Services - Royal North Shore Hospital:
- Provided very significantly erroneous results for homocysteine for Series 1.
- Provided reports with many incorrect sample dates.
- Refused to release the second Series 1a results because I refused to give them an assurance that I will not identify their lab if I publish the results.
Sydney South West Area Health Service - Royal Prince Alfred Hospital:
- Claimed to have 'lost" all 29 of my blood samples.
- Failed to inform me about the "loss" of the samples for two weeks.
- Failed to provide me with any written explanation or apology
